Geriatric Care Specialist
Reports to: Director of Caregiver Services
Qualifications:
- Masters in Social Work or Masters in either Counseling, psychology, or gerontology
- Two years experience in job offered or as a social worker, or any suitable combination of education, training, or experience
Responsibilites:
- Develop and maintain care plans.
- Provide in-home assessment of needs and provide or purchase concrete items to aide to caregivers and care recipients.
- Provide individual counseling and group work for caregivers and senior citizens which may include, but is not limited to: depression, anxiety, bereavement, psychosomatic illnesses, family reconciliation, maladaptive behaviors, cross cultural barriers, stress, or dementia.
- Provide appropriate referral for serious behavioral health issues.
- Provide training/education sessions for caregivers and monthly support group.
- Coordinate provision and purchase of temporary relief for eligible caregivers in the form of congregated or in-home respite services.
- Provide in-home assessment of care recipient to determine ADL eligibility.
- Will also conduct community outreach.
- Will work in our various centers in Manhattan and Queens.
Salary: $46,183 per annum
Hours: 9:00am to 5:00pm; 35/Hr. week
